Job summary

PACE is a health services plan serving adults aged 55 and older with ongoing care needs. This Temp - Social Worker position is based in Garden Grove, CA, within the PACEC Social Work Department.

The role involves 70% care management, 25% support services, and 5% other duties for a duration up to six months. Responsibilities include interdisciplinary team collaboration, care planning, safety assessments, discharge planning, and community resource coordination.

Qualifications

• Master's degree in social work PLUS 2 years of health-related experience.
• 1 year of experience with elderly population required (within last 3 years).
• CPR and First-Aid Certification required or to be obtained within 6 months of hire.
• Valid driver’s license, vehicle or approved transport, and driving record required for 30%+ travel.

Responsibilities

• Participates in IDT initial, 6-month and annual assessments and care planning.
• Documents changes in medical records per standards.
• Assesses high-risk situations and engages in safety planning.
• Leads group counseling and participant support activities.
• Connects participants with community resources and acts as liaison/advocate.
• Supports care coordination needs and warm handoffs with partners.
