Job summary

Habitat Health is seeking a Part-Time Social Worker (MSW) to support elderly participants at our California program. Under the IDT, you will conduct psychosocial assessments, document interventions, and help arrange resources and care transitions.

Under supervision of the Social Services Manager, you will collaborate with primary care providers, lead care conferences, and assist with discharge planning. CA driver’s license and CPR/BLS are required.

Qualifications

• MSW required with 1+ year experience with frail or elderly
• Experience coordinating and facilitating care conferences
• CPR/BLS certification required or obtain within 30 days
• CA driver’s license, transportation, and auto insurance as required by State law
• Immunizations up to date; medical clearance for communicable diseases

Responsibilities

• Conduct social work assessments to determine psychosocial needs, preferences and goals
• Deliver and document social work interventions per care plan
• Arrange resources and services, assist with care transitions, provide counseling
• Collaborate with primary care providers and care team for smooth transitions between settings
• Initiate and facilitate care conference meetings for care coordination
• Provide discharge planning when participants disenroll from the program
