Job summary

The Institute on Aging (IOA)–On Lok PACE program in San Francisco seeks a Medical Social Worker to provide psychosocial assessments and therapeutic counseling for a complex caseload. You will collaborate with an interdisciplinary team to develop and implement participant-centered care plans, coordinating with care managers and nursing staff to ensure safe transitions and optimal outcomes.

Qualifications

• MSW from an accredited school of social work.
• At least one year of professional experience with frail elderly.
• Experience in an interdisciplinary care model preferred.
• Bilingual Cantonese or Spanish preferred.

Responsibilities

• Conduct biopsychosocial assessments to determine participant needs.
• Develop participant-centered care plans integrated with the IDT.
• Coordinate care and transitions with the primary care team and other disciplines.
• Provide supportive counseling for illness, grief, and loss.
• Facilitate family and care team meetings around end-of-life planning.
