We are seeking a compassionate Social Worker (MSW) for a full-time contract position. In this vital clinical case management role, you will serve as an integral member of a team, managing a panel of senior participants and their family caregivers. You will utilize independent judgment to perform comprehensive psychosocial assessments, develop tailored care plans, provide individual counseling, and facilitate family conferences. Additionally, you will act as the primary liaison between participants, community agencies, and medical staff to ensure seamless care coordination and resource referral. Psychosocial Assessments: Conduct initial and ongoing psychosocial evaluations for new and existing participants to identify risks, service needs, and care goals.
Care Planning & Interventions: Formulate, implement, and update individualized care plans in close partnership with participants, family members, and caregivers.
Counseling & Group Support: Deliver individual counseling, facilitate family conferences, and lead participant or caregiver support groups and educational workshops.
Community & Discharge Planning: Connect participants with vital community resources (e.g., DAAS, RCFE, SNF) and coordinate comprehensive discharge planning if a participant dis-enrolls.
Interdepartmental Liaison: Serve as the primary communication bridge connecting senior participants, family support networks, the clinical team, and external agencies.
Team Advocacy: Active participant in regular IDT meetings—advocating for participant needs, establishing care consensus, and supporting unified team treatment goals.
Documentation & Compliance: Complete all clinical documentation, assessment forms, and case notes in a timely, accurate manner according to program standards, legal guidelines, and HIPAA regulations.
Quality Assurance: Participate in continuous quality improvement initiatives to enhance social work department efficiency and service delivery.
Other duties as assigned.
Education: Master’s Degree in Social Work (MSW) from an accredited college or university. (Candidates with a Bachelor's Degree in Social Work (BSW) combined with specialized language skills and extensive geriatric experience may also be considered).
Licensure: Valid Social Worker license/registration in the State of California (or eligible for state practice standards).
Eldercare Specialty: Minimum of one (1) year of experience working with a frail or elderly population.
Clinical Knowledge: Strong background in psychosocial assessment, case management, counseling techniques, and crisis intervention.
IDT Experience: Demonstrated ability to work cooperatively and constructively within a multi-disciplinary team environment.
Technical Skills: Computer literate with working proficiency in Microsoft Office applications and Electronic Health Record (EHR) systems.
Post-MSW Experience: Minimum of one (1) year of post-MSW case management experience in a healthcare or community-based senior program.
Language Skills: Bilingual verbal and written fluency in English and Spanish or English and Cantonese is highly preferred.
Adaptability: Outstanding written and verbal communication skills with the ability to manage shifting priorities in a fast-paced clinical setting.
Work Type: Full-Time Contract Position (40 hours per week).
Work Setting: Hybrid clinical environment, including day center clinics, office spaces, participant homes, and residential care facilities (RCFE/SNF).
Schedule: Standard business hours, Monday through Friday.
