Job summary

Center for Elders' Independence is seeking a Social Worker to support frail and elderly participants in a PACE setting. You will perform psychosocial assessments, develop care plans, and coordinate with an interdisciplinary team (IDT) to address social needs and caregiver involvement.

The role emphasizes comprehensive care planning, family communication, and community resource referrals. Requires a Master’s in Social Work and CA driver’s license, with CPR/First Aid credentials and bilingual

Qualifications

• Master’s degree in social work from an accredited school of social work.
• One year experience with frail/elderly population preferred.
• Ability to work with minimal supervision.
• Excellent oral and verbal communication and meeting facilitation skills.
• Bilingual ability preferred (Spanish, Korean, Cantonese, Mandarin).
• Valid California driver's license with access to reliable transportation.
• Current CPR and First Aid.

Responsibilities

• Develops and implements a comprehensive, integrated plan of care.
• Participates in behavioral management meetings with other mental health professionals.
• Attends psychosocial rounds with consulting psychiatrist; presents cases as indicated.
• Completes initial assessments, re-assessments, and care plan updates.
• Maintains case files, documents progress, and prepares reports.
• Makes referrals to community resources for financial management and benefits.
• Maintains confidentiality per HIPAA principles.
