Job summary

WelbeHealth seeks a Social Worker to manage a caseload of frail seniors, coordinating with an interdisciplinary team to provide comprehensive primary care and social services. The role emphasizes psychosocial assessments, care planning, counseling, and case management to support safe community living.

The position requires MSW completion, strong teamwork, and the ability to coordinate care conferences with the IDT.

Qualifications

• MSW degree completed.
• Minimum of one year of experience with frail or elderly population preferred.
• CPR and First Aid certification within 180 days of hire or upon hire preferred.

Responsibilities

• Conduct psychosocial assessments and participate in IDT meetings to develop care plans.
• Deliver and document social work interventions, arrange resources, assist with care transitions, provide counseling and case management.
• Coordinate with Primary Care Physician and IDT to guide smooth care transitions between settings.
• Initiate and facilitate care conference meetings to ensure high-level coordination among care team and the participant's support network.
• Provide discharge planning when participants disenroll from the program.
