Institute on Aging seeks a Medical Social Worker for On Lok PACE in San Francisco to deliver psychosocial assessments and counseling for participants with complex medical and social needs.

You will join an interdisciplinary care team, coordinate care, advocate for participants, and assist with end-of-life planning. Requires MSW and at least 1 year with frail elderly; Cantonese or Spanish preferred.

Qualifikationen

• MSW from an accredited school of Social Work.
• At least one year of professional experience with frail elderly.
• Experience in an interdisciplinary care model.
• Supervision toward LCSW licensure available, licensure not required.
• Bilingual Cantonese and Spanish preferred.

Aufgaben

• Conducts in-depth biopsychosocial assessments across multiple domains.
• Develops participant-centered care plans with IDT involvement.
• Collaborates with PCPs and RNs to coordinate care.
• Provides supportive counseling for illness, grief, and loss.
• Facilitates family and care team meetings; supports end-of-life planning.
• Engages community partners and enrolls participants in IOA programs.
• Maintains documentation in EHR and ensures confidentiality.
• Participates in quality improvement and data collection.
