Job summary

WelbeHealth, LLC. in California is seeking a Social Worker to join an interdisciplinary team (IDT) serving frail seniors. You will conduct psychosocial assessments, care planning, counseling, and case management to support participants at home and in the community.

You will collaborate with physicians and IDT members, facilitate care conferences, coordinate transitions, and provide discharge planning. A MSW is preferred; CPR/First Aid certification within 180 days is required.

Qualifications

• Minimum of one year of experience working with a frail or elderly population.
• Experience coordinating and facilitating care conference meetings.
• Experience assisting people with behavioral health or substance abuse issues.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Conduct psychosocial assessments and develop participant care plans in IDT meetings.
• Deliver and document social work interventions including resources, care transitions, counseling, and case management.
• Partner with Primary Care Physician and IDT to guide smooth care transitions between settings.
• Initiate and lead care conference meetings to ensure high levels of coordination among the care team and participants’ support networks.
• Provide discharge planning when participants disenroll from the program.
