Job summary

The Center for Elders Independence in California seeks a Social Worker to support frail and elderly PACE participants. You will conduct psychosocial assessments, develop care plans, and facilitate communication among participants, families and the interdisciplinary team.

The role is compensated hourly: $37.97 to $56.95 per hour, with placement based on experience, equity, and market data. Travel to participant sites may be required.

Qualifications

• Master’s degree in social work from an accredited school of social work.
• One year experience working with a frail and/or elderly population preferred.
• Demonstrated ability to work in a multicultural and multidisciplinary team environment.
• Ability to work with minimal supervision.
• Excellent oral and verbal communication and meeting facilitation skills.
• Experience working with severe mental illness is desirable.
• Valid California driver's license, acceptable driving record and insurance with access to reliable transportation.
• Current CPR and First Aid

Responsibilities

• Develops and implements a comprehensive, integrated plan of care as part of the Interdisciplinary Team (IDT).
• Monitors progress toward objectives and communicates changes to team members.
• Attends behavioral management meetings and psychosocial rounds with the consulting psychiatrist.
• Completes initial and re-assessments, and updates care plans.
• Completes chart notes accurately and timely; documents case files.
• Mresents cases, makes referrals to community resources, and coordinates with families.
• Responds to elder abuse, conducts 30-day conferences and annual reviews.
