Zusammenfassung

Senior Community Care of Michigan PACE, a program of Volunteers of America National Services (VOANS), is seeking a Social Worker to join our Lansing, MI team. This role focuses on enrollment assessments, care planning with the Interdisciplinary Team, and ongoing support for seniors living at home.

Qualified candidates hold a Master of Social Work, state licensure or credentialing as applicable, at least one year of experience with frail or elderly populations, and a valid driver’s license.

Qualifikationen

• Masters’ Degree in social work from an accredited school.
• Legally authorized, licensed, registered or certified as applicable in the state of employment.
• Minimum one year of experience working with frail or elderly population.
• Must have a valid driver’s license and transportation.

Aufgaben

• Performs in person initial assessments for enrollment to obtain psychosocial histories and needs.
• Coordinates with the Interdisciplinary Team to develop a plan of care and conducts re-assessments.
• Maintains regular attendance at Interdisciplinary Team meetings and communicates participant changes.
• Provides ongoing support, counseling, and education to participants and family on aging, dementia, grief, and PACE services.
