PACE Southeast Michgan seeks a Master’s Social Worker (MSW) to assess needs, coordinate social services, and deliver care for participants and families in Clinton Township, MI. You will work within the Interdisciplinary Team to plan care using bio-psychosocial assessments and CBT-based interventions.

The role requires MI licensure, at least one year as a clinical social worker, and strong collaboration with caregivers, APS as needed.

Formación

• Master’s Social Worker degree from an accredited school of social work.
• Licensed to practice in Michigan; Limited License may be considered.
• Minimum of one year as a clinical social worker; geriatrics preferred.
• Knowledge of case management, CBT, and bio-psychosocial assessment.
• Familiarity with local, state, and federal regulations; cultural competence.

Responsabilidades

• Conduct initial and follow-up bio-psychosocial assessments.
• Develop and implement individualized treatment plans.
• Coordinate services with care team and discharge planning.
• Conduct risk assessments and crisis intervention as needed.
• Document care plans and communicate with IDT and families.
