Master Social Worker (LMSW or LLMSW) The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. We are seeking a dedicated Social Worker (LMSW/LLMSW) to join our interdisciplinary care team in providing comprehensive services for older adults. In this role, you will coordinate social service programs, conduct in-home assessments, manage community resources, and collaborate with participants' families to enhance their quality of life. The ideal candidate will hold a master's degree in social work, be fully licensed in Michigan, and possess at least one year of experience working with the frail elderly or long-term care population.

What We Require

• Education & Licensing: Must possess a Master's Degree in Social Work (MSW) and be licensed in the State of Michigan.
• Experience: Minimum of one year of experience working with frail elderly or long-term care populations.
• Skills:
  • Detail-oriented with strong problem-solving abilities.
  • Excellent written and verbal communication skills.
  • Ability to maintain discretion and confidentiality.
  • Strong organizational skills and proficiency in MS Office Suite (Word, Outlook, PowerPoint, Excel).
• Certifications: Current CPR certification by AHA, or willingness to obtain it through Senior Care Partners P.A.C.E.
