Job summary

PACE Southeast Michigan in Michigan is seeking a Master’s Social Worker (MSW) to assess needs, coordinate services, and deliver comprehensive social care for participants and families. You will join the Interdisciplinary Team, performing bio-psychosocial assessments and developing care plans with CBT techniques and Motivational Interviewing.

Qualifications

• Master’s in Social Work (MSW) from an accredited school.
• Licensed to practice in Michigan; limited license may be considered depending on experience.
• Minimum of one year clinical social work experience, preferably geriatrics.
• Strong knowledge of case management, short-term interventions, and patient-centered care.

Responsibilities

• Perform comprehensive bio-psychosocial assessments at enrollment and at 180-day intervals.
• Develop and implement individualized treatment plans including CBT and MI techniques.
• Coordinate services with the care team and advocate for participant needs.
• Document thoroughly in the electronic health record and monitor progress.
• Assist with discharge planning and communicate changes to IDT and families.
• Collaborate with the Interdisciplinary Team to revise plans as needed.
