Job summary

PACE Southeast Michgan is seeking a Master’s Social Worker (MSW) to assess, coordinate, and deliver social services for participants and families. The MSW will work within the Interdisciplinary Team (IDT) to develop care plans, educate families, and advocate for participant needs.

Ideal candidates hold an MSW, Michigan licensure, and at least one year of clinical social work experience, preferably in geriatrics, with strong EHR and communication skills.

Qualifications

• MSW from an accredited school of social work.
• Licensed to practice in the State of Michigan.
• One year experience as a clinical social worker, preferably in geriatrics.
• Strong case management and patient-centered care skills.

Responsibilities

• Conduct bio-psychosocial assessments for enrollment and follow-up.
• Develop and implement care plans with the IDT and caregivers.
• Coordinate services and educate participants and families.
• Monitor and document progress in the EHR; adjust plans as needed.
• Advocate for participants and ensure timely communication with the team.
