Volunteers of America’s Senior Community Care of Michigan PACE in Lansing, MI seeks a Social Worker to deliver psychosocial assessments and coordinated care in a healthcare setting. Qualifications

• Master’s Degree in social work from an accredited program.
• Legally authorized, licensed or certified in the state of employment.
• Minimum of one year’s experience with frail or elderly population.
• Valid driver’s license and reliable transportation.

Responsibilities

• Performs in person initial assessments for enrollment and psychosocial history.
• Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care and conducts re-assessments every six months.
• Maintains regular attendance at Interdisciplinary Team meetings and communicates participant changes in the care plan.
• Provides ongoing support, counsel, and education to participants and families on aging, dementia, grief, end of life, and related topics.
