- Minimum of one year of experience with frail elderly or long-term care populations.
- Excellent written and verbal communication; ability to maintain confidentiality.
Responsibilities
- Conduct in-home comprehensive assessments and develop care plans.
- Manage community resources effectively.
- Collaborate with participants' families and facilitate meetings.
- Act as an expert in Advance Care Planning.
- Provide education to alleviate caregiver burden.
