Job summary

PACE Southeast Michgan is seeking a Master’s Social Worker (MSW) to independently assess, coordinate, and deliver social services for participants and families. You will join the IDT, conduct biopsychosocial assessments, and develop individualized care plans including counseling and education.

The role requires Michigan licensure, at least one year of clinical social work experience (geriatrics preferred), and strong case management and EHR/Microsoft Office skills.

Qualifications

• Master’s Social Worker from an accredited school of social work.
• Licensed to practice in Michigan.
• One year experience working as a clinical social worker, preferably in geriatrics.
• Knowledge of case management skills, short-term interventions, and patient-centered care.
• Proficient in Microsoft Word, Excel, Outlook, PowerPoint; ability to learn EHR systems.

Responsibilities

• Conduct comprehensive bio-psychosocial assessments at enrollment and at 180-day intervals.
• Develop and implement individualized treatment plans for participants and caregivers.
• Provide counseling using Motivational Interviewing and CBT techniques.
• Coordinate services with the care team and perform risk assessment and crisis intervention.
• Advocate for participants and coordinate discharge planning activities.
