Job summary

Greenfield Healthcare & Rehab is seeking a compassionate Social Worker to support residents’ physical, emotional, and psychosocial needs in a senior care setting. This role emphasizes holistic care and collaboration with an interdisciplinary team.

Ideal candidates have a BSW or MSW (or related bachelor’s degree) and at least 1 year of social work experience, preferably in long-term care. Strong communication and organizational skills are required.

Qualifications

• BSW or MSW preferred, or a related Bachelor's degree.
• Minimum of 1 year in social work; long-term care experience a plus.
• Strong interpersonal, organizational, and problem-solving skills.

Responsibilities

• Support residents in maintaining physical and emotional well-being.
• Assess and identify residents' social and emotional needs.
• Develop and maintain personalized care plans for residents.
• Coordinate residents' admissions, community re-entries, or transfers to other facilities.
• Participate in interdisciplinary care plan meetings to ensure a holistic approach to resident care.
• Collaborate with the finance team to educate residents and complete necessary documentation.
• Plan and coordinate discharge processes, including arranging DME and home care, ensuring residents' safety when returning to the community.
