Job summary

Fallon Health is seeking a Nurse Practitioner to lead primary care for Summit ElderCare PACE participants, focusing on goal-oriented, person-centered management of chronic and episodic illness in community and long-term care settings.

You will collaborate with the Interdisciplinary Team to develop individualized care plans, address advance directives, and ensure high-quality preventive and wellness care tailored to older adults.

Qualifications

• Master’s degree required in NP program.
• Licensed by MA Board of Registration in Nursing.
• 1–3 years NP experience preferred, geriatric care valued.

Responsibilities

• Performs post enrollment visits and annual exams; documents issues.
• Collaborates with physicians and IDT to manage episodic and chronic care.
• Addresses advance directives discussions annually and with condition changes.
• Maintains and updates care plans with patient goals of care.
• Provides education to participants, caregivers and staff; participates in quality activities.
