Qualifications

• Graduate of an accredited school of nursing.
• Current license as an RN in Massachusetts.
• CPR and Alzheimer’s certification, or willingness to be certified within 60 days of hire.
• At least two years of direct care experience with adults or chronically disabled persons.
• Ambulatory or home care experience is helpful.

Responsibilities

• Reviews and implements Provider’s orders.
• Conducts nursing assessments according to policy guidelines.
• Involves participants and significant others based on needs and abilities.
• Delivers care to participants in any setting based on individualized needs.
• Monitors and evaluates therapeutic interventions.
• Participates in the development and ongoing review of each participant’s care plan.
• Participates in the nursing on-call rotation.
