Job summary

Fallon Health is seeking an experienced Registered Nurse to provide direct nursing care to Summit ElderCare PACE participants in various settings. The role emphasizes patient advocacy, coordination with the interdisciplinary team, and timely, individualized care plans.

The nurse will perform assessments, document care, and participate in training and on-call rotations, while ensuring compliance with policy guidelines and patient safety standards across the service area.

Qualifications

• Graduate of an accredited nursing program.
• Massachusetts RN license; CPR and Alzheimer's certification, or willingness to certify within 60 days.
• At least two years of direct care experience with adults or chronically disabled persons, including care for frail/elderly; ambulatory or home care experience helpful.
• Experience using an electronic medical record system.

Responsibilities

• Provide direct nursing care to PACE participants using nursing process.
• Advocate for the participant's best care and coordinate with the healthcare team.
• Develop and implement individualized plans of care and participate in IDT.
• Document all aspects of participant care and coordinate with team members.
• Participate in on-call rotation and complete required reports.

Education

Graduate of an accredited school of nursing

Massachusetts RN license
