Job summary

Fallon Health, through Summit ElderCare (PACE), provides personal care, light housekeeping, and ADL assistance in participants' homes and at the PACE center. You will follow care plans, work with an interdisciplinary team, and maintain infection control practices.

Under RN supervision, you will document care in the EMR, assist with activities, and contribute to safe, compassionate support for older adults. Pay ranges from $21.50 to $24.00 per hour, weekdays vs weekends.

Qualifications

• Completion of an approved Home Health Aide or CNA Training Program is highly desirable.
• Certification as a Home Health Aide or Nursing Assistant is desired.
• Experience with frail/older adult population; Summit Eldercare competencies annual requirement.

Responsibilities

• Assists with Activities of Daily Living (ADL) needs at the PACE center and in participants' homes.
• Contributes to development of care plans with the Interdisciplinary Team.
• Maintains a clean, safe environment and performs non-skilled treatments.
• Documents care accurately in the EMR per policies.
• Participates in infection control and daily cleaning per CDC guidelines.
