Valir PACE Foundation is seeking an RN Care Coordinator to deliver integrated medical and social services to our participants. You will work with physicians, APRNs, and the IDT to develop and monitor individualized care plans in compliance with CMS regulations.

The role emphasizes comprehensive evaluations, care coordination, and education for participants and families, with opportunity for professional growth and a supportive team environment.

Qualifications

• RN license active in Oklahoma with no restrictions.
• Solid knowledge of federal and state health care laws and CMS requirements.
• Strong written and verbal communication and problem-solving abilities.

Responsabilités

• Perform comprehensive exams and update care plans every six months or with status changes.
• Coordinate with physicians, APRNs, and IDT to monitor care plans for participants.
• Document orders, authorizations, and ensure medication compliance.
• Engage participants and families in medical care decisions and education.
• Maintain accurate patient records and participate in care coordination activities.
