Job summary

Cherry Blossom PACE seeks an experienced RN to deliver comprehensive clinical care and care coordination for participants and prospective enrollees. You will serve as an integrated member of the Interdisciplinary Team, conducting intake assessments and coordinating services across clinic, home, and community settings.

The RN will ensure compliance with federal, state, and CMS PACE regulations while providing participant-centered, high-quality nursing care from intake through ongoing enrollment

Qualifications

Current RN license in good standing.
CPR certification required.
Valid driver’s license for field-based work.
Experience in geriatric, home health, or PACE preferred.

Responsibilities

Provide comprehensive clinical care and care coordination.
Perform intake assessments for participants and enrollees.
Coordinate services across clinic, home, and community settings.
