Job summary

Pace RI is seeking a Nurse Practitioner to provide comprehensive clinical care and care coordination for PACE participants in residential settings across Rhode Island. The NP will assess needs, develop care plans in collaboration with an interdisciplinary team, and support continuity of care across sites.

The role emphasizes transitions of care, medication reconciliation, and communication with facilities, families, and external providers. Regular travel to contracted facilities is required.

Responsibilities

• Provides comprehensive assessment and ongoing management of PACE participants residing in residential care facilities.
• Obtains health histories and performs physical, functional, and mental status assessments; identifies changes and initiates intervention or referral.
• Orders, interprets, and evaluates laboratory, diagnostic, and screening tests and updates the plan of care.
• Prescribes and adjusts medications and maintains accurate medication lists through reconciliation.
• Coordinates transitions of care, including hospital discharges, follow-up and referrals.
• Maintains timely documentation in the electronic health record and ensures HIPAA compliance.
