Job summary

Curana Health is seeking an experienced Psychiatric Nurse Practitioner to deliver high-quality geriatric mental health services across SNF, LTC, AL, and IL communities. You will join a collaborative care team and provide person-centered psychiatric care that supports residents and families.

The role requires travel to assigned facilities, with a focus on continuity across post-acute and long-term care settings; license, DEA, and PMHNP certification are essential.

Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Active DEA license.
• BLS and ACLS certification.
• Current Nurse Practitioner license in the state of practice.

Responsibilities

• Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
• Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
• Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
• Prescribe and administer medications and treatments; provide injections as necessary.
• Educate patients and families on medications, care plans, and preventive mental health practices.
• Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.
