Curana Health, Inc. seeks an experienced Psychiatric Nurse Practitioner (PMHNP) to deliver geriatric psychiatric care across SNF, LTC, AL, and IL communities.

Join a multidisciplinary team to provide high-quality mental health services, with emphasis on continuity of care and patient-centered outcomes. You will travel between assigned facilities, collaborate with physicians and staff, and contribute to a growing value-based care model that supports clinicians and seniors alike.

Qualifications

• Must graduate from an accredited Nurse Practitioner program and hold PMHNP certification (ANCC).
• Active DEA license required to prescribe controlled substances.
• BLS and ACLS certifications required.
• Current Nurse Practitioner license in the practicing state is required.

Responsibilities

• Conduct patient rounds and manage psychiatric care in SNF, LTC, AL, and IL settings.
• Collaborate with attending physicians, consultants, referral sources, and facility staff for coordinated care.
• Assess and manage residents' mental health needs, perform exams, and gather medical histories.
• Order and interpret labs and tests, develop therapeutic care plans.
• Prescribe and administer medications and treatments; provide injections as needed.
• Educate patients and families on medications, care plans, and preventive mental health practices.
• Work with care teams to address acute psychiatric and medical needs promptly.
