Curana Health is seeking a Psychiatric Nurse Practitioner to provide high-quality mental health care across SNF, LTC, AL, and IL communities. You will collaborate with a multidisciplinary team to deliver personalized psychiatric services and ensure continuity of care in the post-acute and long-term care continuum.

Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC PMHNP certification.
• Active DEA license.
• BLS and ACLS certification.
• Current Nurse Practitioner license in the state of practice.

Responsibilities

• Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Partner with physicians, specialists, referral sources, and facility staff for coordinated care.
• Assess and manage residents' mental health needs, perform exams, and review histories.
• Order and interpret labs and tests; develop therapeutic care plans.
• Prescribe and administer medications; provide injections as needed.
• Educate patients and families on medications and care plans.
• Collaborate with care teams to address acute psychiatric and medical needs promptly.
