Curana Health seeks an experienced Psychiatric Nurse Practitioner (PMHNP) to deliver high-quality geriatric mental health services across SNF, LTC, AL, and IL communities. You'll join a multidisciplinary team to ensure continuity of care and personalized psychiatric services.

Qualifications

• PMHNP with ANCC certification.
• Active DEA license.
• BLS and ACLS certification.
• Current NP license in practicing state.

Responsibilities

• Psychiatric care and rounding in SNF, LTC, AL, and IL settings.
• Collaborative care coordination with physicians, specialists, referral sources, and facility staff.
• Patient evaluation and management of mental health needs and medical histories.
• Diagnostics and therapeutics: order tests, diagnose, and plan treatments.
• Medication and treatment management; provide injections as needed.
• Educate patients and families on medications and care plans.
• Address acute psychiatric and medical needs with the care team.
