Job summary

Oak Street Health seeks a Primary Care Physician to deliver equitable value-based care to local Medicare populations in its Chicago-area network of neighborhood centers. Physicians work in a center with large care teams to focus on quality outcomes over volume.

Qualifications

• MD/DO degree or equivalent.
• Board certification in Internal Medicine or Family Medicine (or eligibility).
• Active, unrestricted medical license in the US.
• Active DEA license.
• Authorized to work in the United States.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.

Education

Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate

Internal Medicine or Family Medicine Board Certification (Or board eligible)

Active, non-probationary, unrestricted State License

Active DEA license

US work authorization
