Job summary

Oak Street Health in the Chicagoland area is seeking a Primary Care Physician to deliver equitable, value-based healthcare to local Medicare patients at our neighborhood clinics, supported by integrated care teams to enhance quality over volume. Qualifications

• MD or DO graduate with active medical license.
• Board certification in Internal Medicine or Family Medicine (or board eligible).
• Unrestricted state medical license and DEA registration.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments.
• Maintain Patient EMR data via canopy and greenway.
• Collaborate with regional/central leadership to meet quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.

Education
