Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care for local Medicare populations within our neighborhood centers. You will focus on quality outcomes rather than volume, supported by large care teams to enable you to deliver excellent patient experiences.

New graduates are welcome, with opportunities for leadership as a Center Medical Director for highly qualified candidates.

Qualifications

  • Experience with value-based care models and outcomes-driven care.
  • Experience in outpatient primary care settings.
  • Bilingual proficiency where applicable.

Responsibilities

  • Assess and diagnose patients at our local clinics.
  • Oversee, direct, and administer primary care.
  • Prescribe and administer pharmaceutical treatments and medication.
  • Maintain Patient Electronic Medical Record data.
  • Collaborate with leadership to meet health quality goals.
  • Work with Practice Managers to direct the center care team.
  • Other duties as assigned.
