Job summary

Optimal Care - Evergreen is seeking a Nurse Practitioner or Physician Assistant to provide primary care for geriatric patients. This role emphasizes a patient-centered approach, allowing you to focus on meaningful patient relationships while enjoying a flexible schedule.

The successful candidate will have a Master's degree and a valid license, with at least 2 years of clinical experience preferred. You will benefit from a supportive environment with no weekends or on-call requirements and have the opportunity for month-end bonuses.

Qualifications

• Minimum 2 years clinical experience in a post-acute setting.
• Adult, Family, or Gerontology certification preferred.
• Experience with geriatric patients.

Responsibilities

• Conduct comprehensive medical evaluations and develop treatments.
• Prescribe and monitor medications, coordinating with team members.
• Serve as a key clinical decision-maker for patients.

Skills

Nursing skills as defined by accepted nursing standards

Ability to assess needs of patients, families, caregivers

Ability to handle multiple priorities and documentation

Education

Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant

Current State License as a Nurse Practitioner or Physician Assistant
