Job summary

WelbeHealth in San Bernardino, CA seeks a Primary Care Provider (Nurse Practitioner or Physician Assistant) to lead ongoing primary care for our participants. Work within the WelbeHealth IDT to plan and educate care, with a focus on value-based care and coordinated services.

You will perform initial and semi-annual assessments, help guide discharge planning, and contribute to policy improvements through outcome reviews. Compensation includes base pay plus bonus.

Qualifications

• Graduation from an accredited school of nursing.
• Certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority.
• Minimum of two (2) years of experience working with frail or elderly populations.
• Current knowledge of chronic care, geriatric issues, and best practices.

Responsibilities

• Provide continuous medical coverage to participants, including comprehensive in-person assessments.
• Collaborate with Physicians and IDT members to develop participant care plans and coordinate care.
• Assist in maintaining participant care in skilled nursing facilities and residences with regular visits.
