Job summary

Curana Health is seeking an experienced Psychiatric Nurse Practitioner (PMHNP) to join our geriatric behavioral health team. You will deliver personalized psychiatric services across SNF, LTC, AL, and IL settings, collaborating with a multidisciplinary care team to ensure continuity of care.

Qualifications

• Accredited PMHNP program graduate with ANCC certification.
• Current state NP license and DEA registration.
• BLS/ACLS certified and able to travel to facilities.

Responsibilities

• Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborate with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive care.
• Assess and manage mental health needs, perform physical exams, and gather medical histories.
• Order and interpret labs and tests, develop treatment plans for mental and physical health.
• Prescribe and administer medications and provide injections as needed.
• Educate patients and families on medications, care plans, and prevention.
• Work with physicians and facility teams to address acute psychiatric and medical needs promptly.
