Job summary

InnovAge is seeking a Physician to provide medical care to enrolled geriatric participants across clinics, day centers, homes, and facility settings in coordination with hospital teams. Most visits occur at the InnovAge Center clinic.

Close collaboration with inpatient teams ensures integrated care across settings. Requirements include MD/DO degree, active Florida medical license and DEA, primary care board certification, and at least one year of experience with frail or elderly patients.

Qualifications

• MD/DO degree required.
• Active state medical license and DEA with full prescriptive authority.
• Board certification in primary care required.

Responsibilities

• Provide medical care to participants across settings (clinic, day center, homes, nursing homes, assisted living, personal care homes).
• Document care in medical records and support EMR enhancements as needed.
• On-call coverage after hours on a rotational basis; triaged by RN with limited physician involvement.

Education

Doctor of Medicine or Osteopathy degree

Board Certification in primary care

Medical license and DEA license
