Job summary

InnovAge seeks a Physician to oversee medical care for enrolled geriatric participants across clinics, homes, and care facilities. The role emphasizes coordinating with hospital teams and maintaining high-quality care in a patient-centered model.

Responsibilities include direct medical management, EMR documentation, and participation in interdisciplinary care planning, with after-hours calls triaged by an RN. Competitive benefits and a focus on senior independence are offered.

Qualifications

• Must be a Doctor of Medicine or Osteopathy.
• Current active state Medical License and DEA license with full prescriptive authority.
• Board Certification in primary care specialty.
• Minimum of one year working with the frail or elderly.
• Not on the CMS Preclusion List.

Responsibilities

• Provide medical care to participants, including evaluation, diagnosis, management, and follow-up of conditions.
• Document care in the participant's medical record per PACE policies and EMR enhancements.
• Provide after-hours on-call coverage on a rotational basis; triaged by RN.
• Participate in interdisciplinary care planning and care teams.
• Contribute to quality improvement initiatives within InnovAge PACE programs.
• Present input to initiate new medical programs and processes.
• Provide leadership to foster department morale and a positive culture.
• Perform additional duties as assigned.
