Job summary

PACE by IIH in Stockton, CA is seeking an experienced physician to provide primary medical care to Innovative Integrated Health participants under the Medical Director and alongside the Clinic's interdisciplinary team.

You will assess participants, develop and monitor individualized care plans, order tests, adjust medications, and coordinate specialty referrals while ensuring documentation accuracy and compliance with geriatric care standards.

Qualifications

• Board certified/eligible in internal medicine or family practice.
• Advanced certification in geriatrics preferred.
• Minimum of one year of documented experience working with a frail or elderly population.

Responsibilities

• Provide primary medical care to Innovative Integrated Health participants, including assessment and development of a plan of care with the IDT.
• Develop and monitor individualized care plans in collaboration with the team.
• Perform physical examinations, order and evaluate tests, document results.
• Regulate and adjust medications as needed and facilitate specialist consultations.
• Educate participants and families regarding health maintenance and chronic conditions.
• Provide inpatient attending coverage when hospitalized and communicate status to the team.
• Work with nurse practitioners/physician assistants under supervision at the site.
• Maintain accurate documentation and participate in QAPI activities.
