Job summary

Center for Elders’ Independence is seeking a Primary Care Physician to provide medical care within our PACE program. You will lead an interdisciplinary team and ensure comprehensive care aligned with patient goals, in a collaborative, community-focused setting.

The role requires an MD with a California medical license, experience with frail seniors, and board certification in Internal Medicine or Family Practice; geriatrics preferred. DEA and BLS certifications are required.

Qualifications

• Physician with MD and CA medical license.
• Minimum 1 year of experience with frail or elderly population.
• Board certification in Internal Medicine or Family Practice; geriatrics preferred.
• Current DEA certificate and BLS certification.

Responsibilities

• Provides comprehensive medical care to PACE participants.
• Leads and supports the interdisciplinary team in care planning.
• Reviews and signs care plans every 6 months.
• Supervises CEI nurse practitioners and physician assistants.
• Coordinates referrals, follow-ups, and hospital沟通 with families and care teams.
• Maintains up-to-date medical records in the electronic medical record.
