Job summary

Center for Elders' Independence is seeking a Primary Care Physician to lead medical care for PACE participants under the supervision of the Chief Medical Officer. The PCP will guide the interdisciplinary team and collaborate with clinic leadership to ensure comprehensive medical and non-medical care for participants.

The role requires CA medical license, board certification in Internal Medicine or Family Practice, and experience with frail elders.

Qualifications

• MD degree with valid California medical license.
• Minimum 1 year of experience caring for frail or elderly populations.
• Board certification in Internal Medicine or Family Practice.
• Sub-specialty training in Geriatrics preferred.
• DEA certificate required.
• BLS certification required.
• Commitment to the PACE interdisciplinary team model.

Responsibilities

• Provides comprehensive medical care guided by the PACE benefit.
• Conducts initial, 6-month, annual and periodic assessments.
• Develops and implements participants' care plans with the IDT.
• Reviews care plans every 6 months and coordinates with CEI nurses and PAs.
• Oversees diagnostic tests and medications and arranges follow-up with family.
