Job summary

A PACE center opening in the Stockton, CA area is seeking a Primary Care Physician to join its interdisciplinary care team. Provide comprehensive primary medical care to frail, elderly participants with complex conditions and participate in an IDT with nursing, social work, rehabilitation therapy, and more to plan and coordinate care.

Maintain documentation and participate in on-call coverage as needed. Board-certified or eligible physicians with CA license and DEA registered are preferred to

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine preferred.
• Active, unrestricted California medical license, or ability to obtain prior to start.
• Current DEA registration.
• Experience with geriatric or medically complex populations preferred.
• Prior experience in a PACE program or similar interdisciplinary care model preferred.

Responsibilities

• Provide comprehensive primary medical care to a panel of PACE participants.
• Conduct medical evaluations and develop individualized treatment plans.
• Serve as a member of the interdisciplinary care team (IDT).
• Coordinate participant care across settings and referrals as needed.
• Order and interpret diagnostic testing, prescribe and manage medications.
• Support quality improvement initiatives and CMS/state PACE requirements.
• Participate in an on-call rotation as needed.
