Mattapan Health & Rehabilitation Center is seeking a compassionate Physical Therapist to join our rehabilitation team. You will evaluate residents, develop individualized plans of care, and provide skilled interventions to improve mobility, balance, and independence in a geriatric population.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Current Massachusetts license to practice as a Physical Therapist.
• Current CPR/BLS certification required.
• Long-term care, skilled nursing, or rehabilitation experience preferred.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy interventions to improve mobility and function.
• Collaborate with interdisciplinary team and communicate progress to care partners.
• Maintain accurate documentation and discharge planning where appropriate.
