Job summary

Mattapan Health & Rehabilitation Center is seeking a compassionate Occupational Therapist to join our rehabilitation team. You will evaluate, develop, and implement individualized OT programs to improve function, safety, and independence for residents.

Responsibilities include conducting evaluations, developing treatment plans, and delivering skilled therapies per physician orders. You will educate residents and families, and collaborate with an interdisciplinary team to optimize outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current state license to practice Occupational Therapy.
• Long-term care, skilled nursing, or rehabilitation experience preferred.
• Strong clinical reasoning and treatment planning skills.
• Excellent communication and interpersonal skills.
• Ability to collaborate with residents, families, and interdisciplinary teams.
• Passion for helping residents achieve meaningful functional goals.

Responsibilities

• Evaluate, develop, and implement individualized occupational therapy programs to improve function, safety, and independence.
• Perform OT evaluations and assessments based on resident needs.
• Develop individualized treatment plans focused on ADLs, mobility, cognition, safety and independence.
• Provide skilled occupational therapy treatments according to physician orders and plans of care.
• Help residents improve motor skills, coordination, strength, and functional tasks.
• Incorporate meaningful activities to support resident goals and safety.
• Educate residents, families, and caregivers on strategies to promote independence.
• Recommend adaptive equipment and environmental modifications as appropriate.
• Participate in discharge planning and transition goals.
• Maintain accurate, timely documentation and participate in interdisciplinary meetings.
• Provide clinical direction to OT Assistants and promote a safe rehab environment.
