Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current state license to practice Occupational Therapy.
• Long-term care, skilled nursing, or rehabilitation experience preferred.
• Strong clinical reasoning and treatment planning skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Evaluate, develop, and implement individualized occupational therapy programs to improve function, safety, and independence.
• Perform evaluations and assessments based on resident needs.
• Develop treatment plans focusing on activities of daily living, mobility, cognition, safety, and independence.
• Provide skilled OT treatments per physician orders and plans of care.
• Educate residents, families, and caregivers on strategies to promote independence and safety.
• Participate in discharge planning and interdisciplinary team meetings.
• Provide direction to Occupational Therapy Assistants and maintain documentation.
