Apploi seeks a compassionate Physical Therapist to provide rehabilitation in our facility. You will evaluate residents, develop individualized treatment plans, and deliver therapy to improve mobility, strength, balance, and independence.

Responsibilities include performing exercises, monitoring progress, documenting outcomes, and communicating with nursing staff, physicians, families, and the interdisciplinary team.

Qualifikationen

• Graduate of an accredited Physical Therapy program.
• Current PT license in the state of employment.
• Experience in skilled nursing, long-term care, rehabilitation, or healthcare settings preferred.
• Strong communication and interpersonal skills.
• Compassionate, resident-focused approach.
• Ability to work independently and as part of an interdisciplinary team.

Aufgaben

• Evaluate residents' physical abilities and functional needs.
• Develop individualized treatment plans based on residents' conditions and goals.
• Provide physical therapy treatments according to established care plans.
• Perform therapeutic exercises and functional mobility training.
• Assist residents with improving strength, balance, coordination, and range of motion.
• Monitor residents' progress and modify treatment plans as appropriate.
• Document evaluations, treatments, progress, and other required information accurately.
• Communicate with nursing staff, physicians, families, and other members of the interdisciplinary team.
• Educate residents and caregivers on exercises, mobility techniques, and safety measures.
• Follow facility policies, regulatory requirements, and applicable professional standards.
