Job summary

West Roxbury Health & Rehabilitation Center is seeking a compassionate Physical Therapist to join our rehabilitation team in West Roxbury, MA. You will evaluate residents, develop individualized care plans, and provide skilled PT to improve mobility and independence in a geriatric population.

Join our interdisciplinary team and collaborate with PTAs, nursing staff, and providers while maintaining accurate documentation and contributing to discharge planning and resident progress.

Qualifications

• Graduation from an accredited Physical Therapy program required.
• Current Massachusetts license to practice as a Physical Therapist required.
• CPR/BLS certification preferred or required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy interventions to improve mobility and function.
• Collaborate with PTAs, nursing staff, and interdisciplinary team.
• Document assessments and treatment progress per regulatory standards.
• Participate in discharge planning and resident care planning.
