West Roxbury Health & Rehabilitation Center in West Roxbury, MA is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. You will evaluate residents, develop individualized plans of care, and provide skilled physical therapy to improve mobility and independence.

The ideal candidate holds a valid Massachusetts PT license, CPR/BLS, and has experience in geriatric or subacute rehab. You will collaborate with PTAs and an interdisciplinary team to support resident goals and

Qualifications

Graduation from an accredited Physical Therapy program required.
Current Massachusetts license to practice as a Physical Therapist required.
Current CPR/BLS certification required.
Long-term care, skilled nursing, or rehabilitation experience preferred.
Experience working with geriatric and subacute populations preferred.

Responsabilités

Evaluate residents, develop individualized plans of care, and provide skilled physical therapy interventions designed to improve mobility.
Perform comprehensive physical therapy evaluations and assessments.
Develop individualized treatment plans and measurable therapy goals in accordance with physician orders.
Provide skilled physical therapy treatments within the scope of practice and established plan of care.
Treat residents with orthopedic, neurological, and functional mobility needs.
Collaborate with Physical Therapist Assistants and interdisciplinary teams.
Maintain accurate clinical documentation in accordance with state and federal regulations.
Participate in discharge planning and provide recommendations to support successful transitions.
