Qualifications

• At least one year of experience as a Physical Therapist in acute care, rehab, or home care.
• Current registration with the Massachusetts Board of Registration as a Physical Therapist.
• Strong communication skills—verbal, written, and electronic.
• Comfort with technology and learning new systems.
• Willingness to travel throughout Mass General Brigham Healthcare at Home’s geographic area.

Responsibilities

• Evaluate patients and develop individualized care plans.
• Provide PT focused on mobility, strength, balance, and function.
• Recommend fall prevention and home safety strategies.
• Educate patients and caregivers.
• Track progress and adjust care as needed.
• Collaborate with the interdisciplinary care team.
