Qualifications

• At least one year of experience working with frail or elderly populations.
• Advanced degree in Physical Therapy from an accredited institution.
• Current, active license to practice Physical Therapy in the state of employment (Kentucky).
• Valid driver's license and reliable transportation.
• Immunizations up-to-date and medically cleared for communicable diseases.
• Successful completion of a comprehensive background check.

Responsibilities

• Provide exceptional physical therapy care as an active and collaborative team member.
• Conduct in-person assessments at enrollment and as needed.
• Evaluate and recommend adaptive equipment and home safety modifications.
• Design and implement therapy programs aligned with standards and policies.
• Deliver interventions to achieve individualized goals and document outcomes timely.
• Educate patients, families, and caregivers on home programs and equipment use.
• Maintain licensure, certifications, and participate in trainings.
