Qualifications

• DPT or equivalent degree required.
• Active Physical Therapy license in the state of employment.
• Valid driver's license and reliable transportation.
• Immunizations up to date.
• Background check including criminal history.

Responsibilities

• Provide high-quality physical therapy care as part of a collaborative team.
• Conduct initial and ongoing assessments.
• Recommend equipment and home safety modifications.
• Design and implement therapy programs.
• Document evaluations and outcomes.
• Educate patients and caregivers on home programs.
• Deliver care in clinics, centers, contracted facilities, and patient homes.
• Participate in quality improvement and professional development.
